Travel Registered Nurse (RN) – Home Health / Care Manager

Location: Barre, VT
Contract Duration: 16 Weeks
Start Date: ASAP
Shift: Day Shift
Scheduled Hours: 36 hours/week

Compensation
Pay Rate: $75/hour
Weekly Gross Pay: $2,700/week (including stipend)
Weekly Stipend: $2,107/week
Weekly Taxable Rate: $593/week
Overtime available after 40 hours
Taxable (W-2): $16.40/hour
Non-Taxable: $58.50/hour
Job Overview

We are seeking an experienced Travel Registered Nurse (RN) – Home Health / Care Manager for a 16-week assignment in Barre, Vermont. This position is ideal for an RN with strong home health, care coordination, and patient management experience.

Responsibilities
Provide skilled nursing care to patients in a home health setting.
Assess patient needs and develop appropriate care plans.
Coordinate patient care with physicians, caregivers, and multidisciplinary teams.
Monitor patient progress and communicate changes in condition.
Educate patients and families on treatment plans, medications, and disease management.
Maintain accurate and timely clinical documentation.
Ensure quality, compassionate, and patient-centered care.
Requirements
Active Registered Nurse (RN) license.
Home Health and/or Care Management experience preferred.
Strong clinical assessment and care coordination skills.
Excellent communication and organizational abilities.
Ability to work independently in a home health environment.
