Location: Rutland, Vermont
Contract Duration: 13 Weeks
Scheduled Hours: 37 Hours/Week
Shift: 8:00 AM – 4:00 PM
Schedule Options: Tuesday–Saturday or Sunday–Thursday
Overtime: After 40 hours/week

Job Description
We are seeking an experienced Home Health Registered Nurse (RN) to provide comprehensive, patient-centred care to individuals in their homes. The RN will be responsible for patient assessments, development and implementation of individualized care plans, OASIS documentation, case management, care coordination, patient education, and discharge planning.
The RN will collaborate with physicians, therapists, social workers, LPNs, Home Care Aides, and other members of the interdisciplinary team to ensure safe and effective patient care. The position requires strong clinical judgment, excellent documentation skills, and the ability to work independently in a home health setting.
Responsibilities
• Perform comprehensive patient assessments and ongoing evaluations.
• Complete accurate and timely OASIS documentation.
• Develop, implement, and update individualized patient care plans.
• Provide case management and coordinate services with the interdisciplinary team.
• Perform high-tech nursing services, including IV therapy, wound VAC management, and complex wound care when required.
• Educate patients and caregivers regarding medications, treatments, disease management, and safety.
• Coordinate admissions, transfers, recertifications, and discharge planning.
• Maintain accurate clinical documentation in accordance with agency, state, and federal requirements.
• Supervise and coordinate care provided by LPNs and Home Care Aides.
• Follow infection prevention and control standards.
• Participate in some on-call and holiday coverage as required.
Requirements
• Active RN License in the applicable state.
• Minimum 2 years of Home Health RN experience.
• OASIS documentation experience – Required.
• Case management experience preferred.
• McKesson EMR experience preferred.
• Strong patient assessment, care planning, documentation, and communication skills.
• Experience with IV therapy, wound VAC, and complex wound care preferred.
• Ability to work independently and manage a home health caseload effectively.
• Must comply with agency, state, and federal regulations.
Additional Information
• 37 hours/week guaranteed.
• Holiday coverage and some on-call responsibilities are required.
• Facility does not accept 1099 contractors.
