Job Summary:
The VNA of Albany is seeking dedicated and compassionate Social Workers to join our team. As a Social Worker you will perform services as directed by an authorized Medical Practitioner in the care of the sick and assist in coordination and effective management of patient care services rendered by an interdisciplinary team of workers. Duties/Responsibilities:
• Assesses patient's mental, emotional, socioeconomic, financial, spiritual, cultural, and environmental factors, which affect health status and the person's ability to cope with illness and/or disability.
• Identifies patient problems based on assessment and works with the patient and caregivers to develop, implement, revise and evaluate a Social Work plan for care including other disciplines and community services considering resources available to the patient. Makes appropriate referrals in consultation with the primary Medical Practitioner and primary nurse.
• Evaluates the patient's responses to care and treatment, documents and reports significant changes to the primary Medical Practitioner and primary nurse.
• Utilizes Universal Precautions and appropriate infection control techniques to prevent/control transmission of infection to patients or staff.
• Serves as an interdisciplinary team member, coordinates and communicates plan of care with appropriate disciplines/community resources.
• Maintains the confidentiality of patient medical and financial information. Protects patient records from loss, damage and unauthorized access.
• Assists the health team through consultation and joint visits in evaluation of the patient's and the family's level of psychosocial functioning to estimate the capacity and potential to cope with problems of daily living.
• Maintains timely, accurate documentation of patient care, communication, medical orders, referrals and all other patient care related paperwork. Completes admission paperwork within 1-2 working days.
• Reviews and updates the Home Health Certification and Plan of Care (HCFA 485) every sixty days.
• When possible, prepares in advance for patient discharge so that the patient and those involved in care are aware and have had an opportunity to plan for any continued need for assistance beyond the VNA plan of care. Completes discharge paperwork within seven working days.
• Maintains own professional competence through participation in continuing education and other appropriate learning experiences including current research and literature in the nursing practice.
• Assists with the orientation and/or guidance of new staff members, students, Board members and other selected individuals.
• Maintains awareness of and follows Agency policies and procedures.
• Contributes to the attainment of Agency goals and objectives. Consistently meets productivity standards set by the Department.
• Contributes to the improvement activities of the Agency by participation in Performance Improvement activities and in identification of activities/resources requiring updating and/or further research.
• Acts as a resource and provides supervision for MSW Assistant.
• Acts as a clinical liaison between Intake and community referrals to promote an efficient referral process and effective communication.
• Educates clinical staff in regard to home care services to be provided, assists with Intake coordination and preparation for discharge and directs VNA clinical staff as necessary to promote effective transition to home.
• Establishes relationships with physician groups, assisted living facilities, and nursing homes in specified geographic areas and acts as a clinical liaison with the VNA
• Performs related tasks as requested.
Requirements:
• NY State Licensed Master Social Work (LMSW)
• Driver License / Reliable Transportation
• Home Health experience (Preferred)
• Willingness to travel throughout designated territory
