Description

The Case Manager is responsible for coordinating, planning, and facilitating services for individuals to ensure their healthcare, social, and/or support needs are met. This role involves assessing client needs, developing care plans, monitoring progress, and collaborating with interdisciplinary teams to achieve optimal outcomes.Key Responsibilities
Conduct comprehensive assessments of client needs, including medical, social, and financial factors

Develop, implement, and update individualized care or service plans

Coordinate services across providers, agencies, and community resources

Monitor client progress and adjust plans as needed

Advocate for clients to ensure access to appropriate services and resources

Maintain accurate and timely documentation in compliance with organizational and regulatory standards

Educate clients and families on available resources and care plans

Facilitate communication between clients, families, and healthcare or service providers

Ensure compliance with policies, procedures, and applicable regulations

Participate in interdisciplinary team meetings and case reviews
