LPN/RN Nurse Navigator – Palliative Care

Phoenix Home Care & Hospice is seeking a compassionate and experienced LPN or RN Nurse Navigator to join our Comprehensive Disease Management (CDM) / Palliative Care team in Springfield, MO and surrounding communities.

Schedule: Full Time | Monday–Friday | 8:00 AM–5:00 PM
Location: Springfield, MO and surrounding areas
Salary: $60,000–$70,000 per year

About the Role

The Palliative Care Nurse Navigator helps patients and families navigate complex healthcare needs while receiving support in their homes and communities. This role focuses on care coordination, patient education, goals-of-care conversations, advance care planning, disease management, and connecting patients with appropriate healthcare and community resources.

As a Nurse Navigator, you will help patients understand their options and make informed healthcare decisions that align with their personal goals, values, and care preferences.

You will work closely with patients, families, physicians, specialists, healthcare providers, community partners, and the Phoenix clinical team to promote continuity and coordination of care.

Nurse Navigator Responsibilities

• Conduct consistent face-to-face visits with patients and establish trusting relationships.
• Build rapport with patients and families and initiate goals-of-care conversations.
• Assess patient needs and help identify appropriate healthcare, home care, and community resources.
• Educate patients and families about disease progression, treatment options, palliative care, and available services.
• Assist patients with advance care planning and disease-specific advance directives.
• Coordinate care with physicians, specialists, healthcare providers, and community resources.
• Help patients navigate healthcare decisions, care planning, referrals, and other healthcare needs.
• Support patients and families as they navigate serious and chronic illness.
• Communicate changes, needs, and care plans with the Phoenix clinical team to promote continuity of care.
• Develop and maintain strong relationships with community healthcare partners.
• Complete accurate, timely, and professional clinical documentation following patient visits.
• Collaborate with colleagues to provide coordinated, patient-centered care.

Qualifications

• Current, active Missouri RN or LPN license or Compact RN/LPN license.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Springfield, MO and surrounding areas.
• Strong communication, organization, assessment, and interpersonal skills.
• Ability to work independently while collaborating effectively with patients, families, providers, and clinical team members.
• Hospice or Home Health experience preferred.
• Experience with palliative care, care coordination, chronic disease management, case management, or patient navigation is a plus.
