What You’ll Do
- Provide compassionate hospice nursing care to patients and families
- Educate patients and caregivers on the hospice process and plan of care
- Guide families through goals-of-care discussions and healthcare decisions
- Conduct initial and ongoing comprehensive assessments
- Perform hospice evaluations and informational visits
- Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members
- Coordinate care to ensure patient and family needs are fully met
- Communicate changes in patient condition and update plans of care as needed
What You’ll Need
- Active Missouri or Compact RN license
- Hospice or Long-Term Care nursing experience preferred
- Strong assessment, communication, and care coordination skills
- Valid driver’s license and auto liability insurance
