Licensed Practical Nurse (LPN) Home Health $2,500 Sign On Bonus
Full Time | Monday through Friday | Days

What You'll Do
- Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans.
- Perform comprehensive patient assessments and monitor changes in condition.
- Educate patients and families to promote confidence and independence at home.
- Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team.
- Maintain accurate documentation using our AI-supported charting technology.
- Participate in quality improvement initiatives and ongoing professional development.
Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.
What We're Looking For
- Current Missouri or Compact LPN license.
- Excellent assessment and communication skills.
- Strong organizational and time management abilities.
- Compassionate, patient-centered approach to care.
- Valid driver's license and reliable transportation.
- Home health experience is a plus, but we welcome nurses who are eager to learn.
