We are currently looking for a Licensed Practical Nurse (LPN) to join our team.

Licensed Practical Nurse (LPN) Skills and Qualifications:
LPN (Licensed Practical Nurse) licensed by the State
CPR Certification required
Two Years prior nursing experience preferred
Valid NYS LPN license
Previous experience in long-term care or a skilled nursing facility is preferred
Strong communication and organizational skills
Compassionate and dependable with a team-first attitude
Licensed Practical Nurse (LPN) Responsibilities
Admissions, Discharge and Transfers of residents
Medication requisitions with pharmacy and physicians
Medication administration
Following infection control, emergency, and other center policies
Collaborating with resident’s interdisciplinary team to provide the best outcomes
Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
