Connected Health Care is seeking an experienced and motivated Physical Therapist (PT) for an exciting 13-week travel assignment in Los Angeles, California. Job Description:

• Evaluate and treat patients to improve mobility and reduce pain

• Develop personalized therapy plans and track patient progress

• Collaborate with a multidisciplinary care team

• Educate patients and families on treatment goals and exercises

Requirements:

• Active PT license

• California licensure (or ability to obtain)

• Degree from an accredited Physical Therapy program

• Prior clinical experience preferred, especially in rehab or skilled nursing settings

• Strong communication and documentation skills
