Job Requirements
  • Education Requirements: Bachelor’s degree in Nursing, Healthcare Administration, or related field
  • Certifications Requirements: BLS certification; Valid driver’s license; Auto insurance
  • Experience Requirements: Minimum of 2 years of hospice or home health experience; Prior case management experience preferred; Familiarity with EMR systems such as Epic and Rover
  • Other Requirements: Ability to work independently; Strong communication and organizational skills; Valid driver’s license and reliable transportation

Job Responsibilities
  • Provide comprehensive hospice case management services to end-stage patients in home, assisted living facilities, skilled nursing facilities, and hospitals.
  • Assess patient needs, develop individualized care plans, and coordinate interdisciplinary team efforts to ensure quality patient outcomes.
  • Monitor patient progress through home visits and telehealth interactions, adjusting care plans as necessary.
  • Maintain detailed documentation of patient interactions, treatments, and progress within EMR systems such as Epic and Rover.
  • Educate patients and their families about end-of-life care, symptom management, and available resources to promote comfort and dignity.
