- Certifications Requirements: BLS certification; Valid driver’s license; Auto insurance
- Experience Requirements: Minimum of 2 years of hospice or home health experience; Prior case management experience preferred; Familiarity with EMR systems such as Epic and Rover
- Other Requirements: Ability to work independently; Strong communication and organizational skills; Valid driver’s license and reliable transportation

- Provide comprehensive hospice case management services to end-stage patients in home, assisted living facilities, skilled nursing facilities, and hospitals.
- Assess patient needs, develop individualized care plans, and coordinate interdisciplinary team efforts to ensure quality patient outcomes.
