Job Requirements- Education Requirements: Valid Registered Nurse (RN) license in Wisconsin
- Certifications Requirements: BLS certification; Valid Driver’s License; Auto insurance
- Experience Requirements: Minimum of 2 years of hospice or home health experience; Experience managing end-stage patient cases; Familiarity with EMR systems, preferably Epic and Rover
- Other Requirements: Ability to support home health supplies; Strong communication and teamwork skills; Flexibility to support community-based patient care
