Contract Length: 13 Weeks
Weekly Compensation: Up to $3,160 (Based on Experience)

Connected Health Care is seeking an experienced Physical Therapist (PT) for a local travel assignment in Hollister, California. Job Description:

• Evaluate, plan, and implement individualized treatment plans for patients
• Use evidence-based practices to restore function, relieve pain, and improve mobility
• Educate patients and families on home exercise programs and prevention strategies
• Collaborate with physicians, nursing staff, and other therapists to ensure quality care
• Document patient progress and update treatment plans accordingly
• Maintain compliance with all local, state, and federal regulations

Education & Certification Requirements:

• Degree in Physical Therapy from an accredited program – Required
• Current California PT License – Required
• BLS Certification – Required
• Minimum of 1 year of clinical experience preferred
