Connected Health Care is seeking an experienced and motivated Home Health Physical Therapist Assistant (PTA) for a 13-week local travel assignment based in Independence, MO. This opportunity is with one of our trusted healthcare partners and is ideal for clinicians who excel in a home health setting and are comfortable working independently while supporting high-quality patient care.

The ideal candidate will have a strong home health background and be comfortable assisting with patient care plans, documentation, and traveling throughout the assigned Missouri service area.

Job Responsibilities

• Provide skilled physical therapy assistance to patients in their homes across Missouri service areas
• Implement treatment plans established by the supervising Physical Therapist
• Assist with patient mobility, therapeutic exercises, and functional training
• Monitor patient progress and report changes to the supervising PT
• Document patient visits accurately and in a timely manner
• Educate patients and caregivers on prescribed home exercise programs
• Maintain compliance with all home health regulations and standards
• Work independently while ensuring high-quality patient care

Education & Certification Requirements

• Associate degree from an accredited Physical Therapist Assistant program
• Active Physical Therapist Assistant (PTA) license in Missouri required
• Current CPR/BLS certification
• Minimum of 1–2 years of home health experience preferred
• Experience with home health documentation systems preferred
