Join our team as a Physical Therapist (PT) in Home Health, serving the vibrant community of Oakland, CA, in Alameda County. Role Responsibilities:

• Prioritize start of care visits
• Supervise Physical Therapy Assistants
• Complete mandatory reporting of start of care visits by the next day
• Submit clinical notes and other documentation within 48 hours of each visit
• Maintain timely communication with supervisor, intake, and office staff
• Be responsive during normal business hours

Requirements:

• California PT License
• One year of Home Health experience

Schedule: 40 hours per week, Monday-Friday with one weekend rotation per month.

Training: 3 full days, including one in-house and two in the field.

Documentation System: Kinnser

Equipment Issued: iPad, Verizon One Talk

Mileage Reimbursement: $0.65/mile (from first to last patient)

Parking and Toll Reimbursement: Paid

Company Car: Option available with a decreased bill rate

Overtime Rate: +$5, paid after 30 points per week

Holiday Rate: +$10

Productivity Requirement: 30 points per week (1 point = 1.33 hours)

Caseloads and visit mixes per day will average 8 hours per day and 40 hours per week. Overtime must be pre-approved by the Clinical Supervisor.
