NEW Grads Welcome!! A leading healthcare provider is looking for a qualified Occupational Therapist. Home health experience is preferred but not required. The assigned county territory includes Area #6 (Vacaville, Fairfield, Rio Vista, Suisun City, Dixon) and Area #7 (Vallejo, Benicia, Napa, American Canyon, Yountville).

There are TWO options available: Guaranteed Hours or Non-Guaranteed Hours. If you choose the Guaranteed option, the following shall apply:

• Productivity: At least 27 points per week (points described below).
• 36 hours Guarantee.
• You must complete 30 points in the week.
• Willingness to travel outside the area or work weekends may be required to maintain weekly visit expectations.
• If you refuse patients, the guarantee does not apply.
• The guarantee does not apply for the 1st and last week of the assignment, or for weeks in which there are holidays, or for weeks in which you have time off.

If you choose the Non-Guarantee option, the following shall apply:

• Weekend coverage may be required.
• To secure a full caseload, you may be required to go outside the service area occasionally.
• You can refuse patients.
• No guaranteed hours.
• You must be willing to see Covid+ and Covid PUI patients.
• Payment is made on a per visit basis.

The visit types and their conversion to points and hours are as follows:

• OASIS ReCertification and OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours.

Although Travelers are paid hourly by their agency, hours are approved by the healthcare provider on a per visit basis. Note that the rate is all-inclusive and covers drive/travel time, visit time, documentation time, and time spent in making phone calls to patients and doctors.

Credentials Required: Current CA OT License, BLS, Driver's License, Auto Insurance, Auto Registration, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical.

Your agency's contract to you must mirror the confirmation page. If your agency does not comply with this standard, your agency will be at risk of being removed from this client's vendor set. No exceptions!

• Start Date Must be Identical.
• End Date Must be Identical.

***Please Note - All Requested Time Off (RTO) must be disclosed at the time of submission. If your agency or you do not include all RTO in the submission form and adds it post-confirmation, your agency will be at risk of being removed from this client's vendor set. No exceptions!

Time off requested after the acceptance date will not be approved.
