Join our team as a Case Manager Registered Nurse at a leading healthcare facility in Eureka, CA. This contract position offers an opportunity to work in a dynamic environment where your skills will make a significant impact on patient care.
Key Responsibilities:
- Manage patient care plans and coordinate services.
- Work collaboratively with healthcare teams to ensure quality care.
- Document patient progress and adjust care plans as needed.
- Provide education and support to patients and their families.
Requirements:
- Active RN license in California.
- Minimum of 2 years of acute care experience.
- First-timers accepted with clinical approval.
- Weekend work required every other weekend.
- Ability to float between like units based on need.
- Charting experience with Epic is preferred.
- All RTO must be included at the time of submittal.
