Unit Information:

• Number of Beds: Units are mainly 20 beds each.
• Number of Staff: Team of Case Managers and Social Workers to manage the patient load.
• Patient Ratios: Social Worker: 1:40, RN: 1:20.
• Type of Equipment: Mobile, computer.
• EMR: Meditech / CarePort.

Typical Hiring Profile:

• Must Have: Minimum 1-year current Case Management RN experience.
• Preferred: Discharge Planning, ED experience, Palliative Care Knowledge, and GOC Conversation.

Typical Procedures Performed:

• Discharge Planning Assessments.
• Discharge Planning.
