Description

Seven Healthcare is seeking an experienced Case Management RN for a respected acute‑care facility in Largo, Florida. This day‑shift contract is ideal for an RN skilled in discharge planning, utilization review, and coordinating safe, effective transitions of care.

Key Responsibilities

Coordinate comprehensive discharge plans and transitions of care
Perform utilization review and ensure payer‑compliant documentation
Collaborate with physicians, nursing, therapy, social work, and external partners
Educate patients and families on care needs, equipment, and community resources
Maintain clear, detailed documentation in EMR systems
Support interdisciplinary case conferences

Schedule Details

5×8‑hour day shifts
40 hours/week
13‑week contract

Qualifications

Active RN license (FL or compact)
Minimum 1 year recent Case Management or Utilization Review RN experience
Strong communication, assessment, and care‑coordination skills
BLS required
