Seven Healthcare is seeking a dependable Case Management RN for a reputable hospital in Fort Walton Beach, Florida. This day‑shift assignment is perfect for an RN skilled in coordinating individualized plans of care while ensuring safe and timely patient transition.

Key Responsibilities

Perform case management assessments and build personalized care plans
Communicate with multidisciplinary teams to support quality care
Ensure documentation meets hospital and payer requirements
Educate patients/families regarding discharge resources and follow‑up
Facilitate safe transitions across care settings
Participate in unit‑based care coordination huddles

Schedule Details

5×8‑hour day shifts
40 hours/week
13‑week assignment

Qualifications

Active RN license (FL or compact)
Minimum 1 year hospital Case Management RN experience
BLS required
Strong knowledge of utilization review & discharge planning
