Description

Seven Healthcare is seeking an experienced Case Management RN for a well‑respected acute‑care hospital in Brownsville, Texas. This day‑shift assignment is perfect for an RN skilled in discharge planning, utilization review, and coordinating comprehensive patient care transitions.

Key Responsibilities

Coordinate safe and effective transition‑of‑care plans
Perform utilization review and ensure compliance with payer requirements
Collaborate with physicians, nursing, therapy, and social work
Conduct patient/family education regarding care needs and resources
Maintain accurate documentation in the EMR
Support continuum‑of‑care processes across hospital and community settings

Schedule Details

5×8‑hour day shifts
40 hours/week
13‑week contract

Qualifications

Active RN license (TX or compact)
Minimum 1 year recent Case Management or Utilization Review RN experience
Strong care‑coordination and communication skills
BLS required
