Seven Healthcare is pleased to offer a travel opportunity for an RN – Case Manager (Home Health) in Jeffersonville, IN. This assignment consists of 8-hour day shifts, five days per week, totaling 40 hours, with a weekly pay of $2,600.

Key Responsibilities of the RN – Case Manager (Home Health):

Conduct comprehensive in-home patient assessments

Develop, implement, and manage individualized care plans

Coordinate care with physicians, therapists, and interdisciplinary teams

Monitor patient progress and revise care plans as necessary

Provide patient and family education on medications and chronic condition management

Complete accurate and timely OASIS documentation

Ensure compliance with Medicare, Medicaid, and agency standards

Promote patient safety, independence, and quality care in the home setting

Requirements:

Active Indiana RN license

Minimum 1–2 years of recent home health and/or case management experience

Current BLS certification

OASIS documentation experience preferred

Strong clinical assessment, communication, and organizational skills

Valid driver’s license and reliable transportation required
