Description

Seven Healthcare is pleased to offer an outstanding opportunity for an experienced Travel Case Management Registered Nurse in Bradenton, Florida. This assignment includes a weekly blended rate of $2,320, based on five 8‑hour day shifts each week. This role is ideal for an RN who excels in discharge planning, care coordination, utilization review, and ensuring patients transition safely and appropriately across levels of care.

Key Responsibilities of the Case Management RN

Conduct initial and ongoing assessments to identify discharge needs
Develop individualized care plans to support safe transitions of care
Coordinate with physicians, social workers, physical therapists, and insurance representatives
Facilitate access to home health, rehabilitation, SNFs, outpatient follow‑up, and community resources
Perform utilization management and ensure appropriate length‑of‑stay
Document all case management activities accurately and timely
Communicate clearly with patients and families regarding treatment plans, discharge expectations, and available resources

Requirements of the Case Management RN

Active Florida RN license or compact eligibility
Current BLS certification
Minimum 2 years’ recent Case Management or Care Coordination experience
Strong communication and organizational skills
