RN Case Manager Qualifications

• Active Registered Nurse (RN) license in Pennsylvania

Homecare RN Responsibilities

• Coordinate and deliver high-quality home health nursing care to patients in Plainsboro, West Windsor, and the greater Philadelphia area
• Develop, implement, and evaluate individualized patient care plans as a case manager
• Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive clinical support
• Educate patients and families on health management, disease prevention, and homecare best practices
• Maintain accurate and timely documentation of patient progress and nursing care provided
• Ensure compliance with all regulatory and safety standards in home healthcare
