Registered Nurse Qualifications
- Active Kentucky RN license
- Minimum 2 years of recent experience as a Home Health Registered Nurse
- Proficient in OASIS and HCHB (Homecare Homebase) documentation systems
- Current CPR certification
- Valid driver’s license, vehicle insurance, and reliable transportation
Home Health Nursing Responsibilities
- Provide patient-centered nursing care in home health settings
- Perform comprehensive patient assessments and develop individualized care plans
- Document patient care accurately using OASIS and HCHB systems
- Collaborate with interdisciplinary healthcare teams for continuity of care
- Educate patients and families on health management, preventive care, and chronic disease management
- Travel within the La Grange, KY area to deliver clinical support to patients
