• Pay Rate: $58 per hour (W2)

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Minimum 1 year of nursing experience (home health care or related clinical experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage a flexible schedule
• Valid driver’s license and reliable transportation for home health visits

Home Health RN Responsibilities

• Deliver high-quality nursing care to patients in their homes throughout Montgomery County and Northwest Philadelphia
• Conduct comprehensive patient assessments, develop individualized care plans, and monitor patient progress
• Administer medications, perform wound care, and provide other prescribed clinical treatments
• Educate patients and families on health management, chronic disease care, and preventive healthcare
• Collaborate with interdisciplinary healthcare teams to ensure continuity of patient care
• Accurately document all patient interactions and nursing care provided in compliance with healthcare regulations
