• Shifts per Week: 5
• Shift Duration: 8-hour days
• Estimated Weekly Pay: $2,270 – $2,390

Job Requirements / Qualifications

• Active Registered Nurse (RN) license in the state of Pennsylvania (PA)
• Minimum 1 year of nursing experience; home health/homecare experience preferred
• Current BLS certification (AHA)
• Strong clinical assessment, patient education, and communication skills
• Reliable transportation and valid driver's license for home visits
• Experience with electronic medical records (EMR) and thorough documentation preferred

Responsibilities

• Provide compassionate, high-quality nursing care to patients in their homes as a homecare RN
• Develop, implement, and update individualized care plans based on clinical assessments
• Monitor patient progress, manage symptoms, and coordinate care with interdisciplinary teams
• Educate patients and families on disease management, medication administration, and preventive care
• Perform home health nursing duties including clinical assessments, wound checks, and medication reconciliation
• Document all care provided in accordance with regulatory and agency standards using EMR
