Job Requirements

• Minimum 2 years of home health nursing experience required — experienced homecare RN or home health nurse preferred
• Active Registered Nurse (RN) license in the state of Pennsylvania (PA)
• Proven case management experience and strong OASIS documentation skills
• Wound care experience preferred (skilled nursing/wound management)
• Ability to manage a high-volume caseload (typically 5–6 patients per day)
• Excellent organizational, clinical assessment, and documentation skills

Responsibilities

• Provide comprehensive home health case management for patients in Plainsboro, West Windsor and surrounding communities
• Conduct thorough patient assessments, develop individualized care plans, and coordinate care delivery with the interdisciplinary team
• Complete accurate and timely OASIS documentation to meet regulatory and quality standards
• Deliver skilled nursing services, including wound care, medication management, and patient education
• Manage a high-volume caseload while maintaining patient-centered care and positive outcomes
• Collaborate with physicians, therapists, and other home health providers to ensure continuity of care
• Educate patients and families on care plans, disease management, and safe home health practices
