Job Requirements / Qualifications

• Active Registered Nurse (RN) license (state-appropriate licensure required for travel nursing assignment)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Epic EMR experience (electronic medical record proficiency)
• Minimum 1 year of bedside clinical experience (2 years highly desired) — LTAC or acute care experience preferred

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission to develop individualized long-term acute care plans
• Collaborate with interdisciplinary teams and families to identify patient teaching needs and rehabilitation goals
• Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
• Document assessments, interventions, and patient responses accurately and in a timely manner in Epic EMR
• Participate in patient rehabilitation programs and initiate discharge planning from admission
• Communicate changes in patient status to providers to maintain continuity of care and patient safety
• Supervise and provide direction to ancillary nursing personnel and support staff
• Promote patient and employee safety by adhering to hospital policies, procedures, and LTAC clinical protocols
• Contribute positively to team building and a supportive work environment on the night shift
• Assume charge responsibilities as needed after orientation
• Engage in ongoing professional development and continuing education relevant to long-term acute care nursing
