Job Requirements

• Active Registered Nurse (RN) license in the state of Pennsylvania
• Minimum 1 year of nursing experience; home health / homecare experience preferred
• Familiarity with OASIS documentation and home health assessments is highly desirable
• Strong communication, organizational skills, and ability to manage a patient caseload independently
• Experience with care coordination and interdisciplinary team communication (case management skills a plus)
• Current BLS certification from an accredited provider (AHA preferred)

Responsibilities

• Provide skilled nursing care to patients in their homes, following individualized care plans (home health nursing)
• Conduct comprehensive patient assessments and complete OASIS documentation accurately and in a timely manner
• Coordinate care with interdisciplinary team members, physicians, and community resources to ensure optimal patient outcomes
• Provide patient and family education on disease processes, medication management, and home care techniques
• Monitor patient progress, update care plans as needed, and communicate changes to the care team
• Maintain accurate clinical documentation and ensure compliance with home health agency policies and Pennsylvania regulations
