Job Requirements

• Active RN license in Pennsylvania (PA RN)
• Minimum 1 year of nursing experience; homecare or home health experience preferred
• Ability to work independently and manage a caseload (case management skills)
• Strong communication and organizational skills
• Valid driver's license and reliable transportation for community/home visits

Responsibilities

• Provide skilled nursing care to patients in their homes (home health nursing, homecare)
• Assess patient needs and develop individualized care plans based on clinical findings
• Administer medications, wound care, and other treatments as prescribed
• Collaborate with interdisciplinary team members and home health agency staff to ensure optimal patient outcomes
• Educate patients and families on health management, self-care techniques, and discharge planning
• Document all patient interactions and care provided accurately and in a timely manner
