Job Requirements

• Current, active RN license in Pennsylvania (Registered Nurse)
• Minimum 1 year of nursing experience; home health or homecare experience preferred
• Current BLS certification (AHA preferred)
• Strong clinical assessment, care coordination, and communication skills
• Ability to work independently and safely in a homecare environment

Responsibilities

• Provide skilled nursing care to patients in their homes, ensuring safety, comfort, and patient-centered care
• Assess, plan, implement, and evaluate individualized patient care and home health nursing plans
• Collaborate with interdisciplinary teams, caregivers, and families to update and coordinate care plans
• Educate patients and families on diagnoses, medications, wound care, and self-care techniques
• Document all patient interactions, treatments, and clinical observations accurately and in a timely manner
• Maintain compliance with regulatory, clinical, and organizational standards for home health services
