- Position: Travel Occupational Therapist (OT) – Hospital & Home Health (OT travel assignment)
- Weekly Estimated Pay: $1,900 – $2,040
- Location: Rockford, Illinois (61104) — Illinois occupational therapy jobs / Rockford OT jobs
- Setting: Hospital and Home Health (home health OT, hospital-based OT)
- Employment Type: Travel Contract (13 weeks)
- Start Date: April 27, 2026
- End Date: July 25, 2026
- Shift: 8-hour days, Monday–Friday (8:00am–5:00pm), rotating weekends
- Guaranteed Hours: 40 hours per week
- Mileage: Mileage between home health visits paid
Qualifications
- Active Illinois Occupational Therapist license (OT license in IL) required
- Minimum 1 year of experience as an Occupational Therapist, with home health experience preferred
- BLS (Basic Life Support) certification required
- Experience with patient assessments, care planning, therapeutic interventions, and ADL training
- Travel candidates only — no local applicants within a 50-mile radius
- Comfortable with documentation in electronic medical records (EMR) and accurate clinical charting
Key Responsibilities
- Provide occupational therapy assessment and treatment to adults in their homes and in the hospital setting
- Develop and implement individualized care plans focused on ADLs, mobility, adaptive equipment, and safety
- Deliver patient-centered therapeutic interventions and education to patients and families
- Collaborate with interdisciplinary healthcare teams (nurses, PT, SLP, social work, physicians) to optimize patient outcomes
- Perform home safety evaluations and recommend adaptive strategies and durable medical equipment as needed
- Maintain timely, accurate documentation of patient progress, outcomes, and plan-of-care in the EMR
- Participate in rotating weekend coverage as required by the travel assignment
