Job Requirements

• Active RN license in Pennsylvania (required)
• Minimum 1 year of nursing experience; homecare or home health experience preferred
• Strong clinical assessment, triage, and case management skills
• Ability to work independently, manage multiple homecare cases, and prioritize visits
• Excellent communication, documentation, and organizational skills

Responsibilities

• Provide home health nursing and RN case management for patients in Southwest Philadelphia
• Conduct comprehensive in-home assessments and develop individualized care plans
• Monitor patient progress, perform clinical reassessments, and adjust care plans as needed
• Coordinate care and collaborate with interdisciplinary teams, primary care providers, and families to ensure optimal outcomes
• Document all patient interactions accurately and maintain thorough medical records
