Weekly Estimated Pay: $2,120 - $2,230
Location: Scranton, PA (zip code 18508) — serving Luzerne County and Northeastern Pennsylvania
Position Type: Travel, Home Health RN / Homecare RN
Hours per Week: 40 (weekly hour guarantee: 32)
Schedule: Monday–Friday, 8:00am – 4:30pm (8-hour shifts)
On-Call/Weekend: Every 3rd weekend; 1 night on-call per week
Dress Code: Scrubs (no specific color required)
Mileage Reimbursement: Yes — reimbursed between patient visits
Contract Duration: Through December 31, 2026

Qualifications and Requirements

Active Registered Nurse (RN) license in Pennsylvania
Associate's Degree in Nursing (ADN) or higher
Minimum 1 year of nursing experience; recent home health experience preferred
Experience with HCHB (Homecare Homebase) is a plus
Professional certification required
Valid driver’s license and ability to travel within assigned territory

Key Responsibilities

Serve as RN case manager for an assigned territory, providing comprehensive home health nursing care
Perform skilled routine visits, recertifications, discharges, and start/resumption of care
Provide 100% patient care in the home setting—assessments, care planning, patient/family education
Document accurately in electronic health records (EHR) and follow agency policies—HCHB experience preferred
Collaborate with interdisciplinary team and other case managers to ensure continuity of care and coverage during days off
Meet productivity expectations (30 points per week) and participate in scheduled on-call rotation and weekend coverage
