Homecare Registered Nurse (RN) – Travel Contract in Uniontown, OH. Seeking an experienced Home Health RN with OASIS and HCHB skills for a 13-week travel nursing assignment covering Portage, Medina, and Summit Counties. Join our home health team and deliver high-quality in-home care across Northeast Ohio.

Job Details

• Weekly Estimated Pay: $2,120 - $2,230
• Location: Uniontown, OH 44685 (Coverage Areas: Portage County, Medina County, Summit County)
• Employment Type: Travel Contract / Travel RN / Home Health Job
• Duration: 13 weeks (April 20, 2026 – July 19, 2026)
• Guaranteed Hours: 40 hours per week
• Shift Schedule: Monday–Friday, 8:00 AM – 4:30 PM
• On Call / Weekend Requirement: 1 weekday evening per week (4:30 PM – 8:00 AM) and 1 weekend per month (Fri 4:30 PM – Mon 8:00 AM)
• Shift Duration: 8 hours
• Dress Code: Black, Grey, Navy Blue Scrubs
• Mileage Reimbursement: Yes (between visits from first patient to last patient)

Required Qualifications

• Minimum 1 year of homecare / home health RN experience (Home Health Nurse)
• Proficient in OASIS assessments and HCHB (Homecare Homebase) documentation
• Associate's Degree in Nursing (ADN) or higher
• Active Registered Nurse (RN) license in Ohio
• Current CPR certification
• Clinical skills: wound care, wound vac management, ostomy care, blood draws, PICC/IV care, Foley catheter care, and medication management
• Strong communication, adaptability, and patient/family education skills

Key Responsibilities

• Provide skilled nursing care and clinical assessments in patients' homes across Portage, Medina and Summit Counties (home health nursing)
• Complete and submit OASIS assessments and documentation using HCHB software (electronic home health documentation)
• Perform skilled procedures including wound care, wound vac changes, ostomy care, blood draws, PICC/IV maintenance, and Foley care
• Manage medications, administer treatments, and teach patients and caregivers safe care and medication management
• Participate in on-call rotation for weekday evenings and scheduled weekends
• Meet productivity expectations (30 points) while maintaining high-quality patient outcomes
• Collaborate with interdisciplinary teams (physicians, therapists, case managers) to coordinate care plans and ensure continuity of care
