Job Requirements / Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Minimum 1 year of clinical experience in home health, homecare nursing, or a related setting
• Experience conducting OASIS assessments, developing care plans, and completing return visits
• Proficiency in IV therapy, wound vac management, advanced wound care, and venipuncture
• Associate’s Degree in Nursing (ADN) or higher
• Strong communication, patient education, and organizational skills
• Reliable transportation and ability to travel between patient homes (mileage reimbursed)

Primary Responsibilities

• Perform comprehensive OASIS assessments and document findings for home health patients
• Develop and implement individualized care plans and complete timely return visits
• Administer IV therapy, manage wound vacs, and provide advanced wound care interventions
• Perform venipuncture and other nursing procedures as required
• Provide patient and family education on disease management, medication administration, and self-care
• Accurately document all care, communications, and clinical progress in the electronic health record
• Collaborate with interdisciplinary home health team members to optimize patient outcomes
