Job Details

• Weekly Estimated Pay: $2,120 - $2,230
• Employment Type: Travel / Homecare RN (Travel Nurse)
• Location: On-site in Port Angeles, WA 98362 (serving Clallam and Jefferson counties)
• Shift Schedule: Monday–Friday, 8:00 AM – 4:30 PM
• Shift Duration: 8 hours per shift
• Hours per Week: 40 (32 hours guaranteed)
• Contract Length: 14 weeks (April 27, 2026 – July 31, 2026)
• Number of Positions: 2
• Coverage Area: Home visits across Clallam County and Jefferson County
• Dress Code: Solid color scrubs
• On-Call/Weekend: Occasional on-call or weekend coverage required for call-offs
• Travel: Mileage reimbursed between patient visits

Qualifications & Requirements

• Active Washington RN license (required)
• Associate Degree in Nursing (ADN) or higher
• Minimum 1 year experience as a hospice RN case manager or hospice/home health RN
• Professional hospice certification required (per agency policy)
• Experience with Homecare Homebase (HCHB) or similar EMR is a plus
• Strong clinical assessment, care planning, and documentation skills
• Valid driver's license and reliable transportation for home visits

Key Responsibilities

• Provide RN case management and skilled nursing care for hospice patients in their homes
• Coordinate and deliver compassionate end-of-life care with a patient- and family-centered approach
• Develop, implement, and update individualized hospice care plans in collaboration with interdisciplinary teams
• Accurately document patient assessments, interventions, progress notes, and outcomes in EMR (HCHB preferred)
• Manage symptom control, medication administration, and patient education during home visits
• Travel between patient homes across Clallam and Jefferson counties; mileage reimbursed
• Participate in occasional on-call and weekend coverage when needed
