• Hours per Week: 40 (5 shifts/week, 8-hour days)
• Weekly Estimated Pay: $2,270 – $2,390

Required Qualifications

• Active Registered Nurse (RN) license in the state of Pennsylvania
• Minimum 1 year of nursing experience; home health or homecare nursing experience preferred
• Strong clinical assessment, care planning, and critical thinking skills
• Excellent communication and interpersonal abilities for patient and family education
• Ability to work independently, manage time, and prioritize caseloads
• Valid driver’s license and reliable transportation for in-home visits
• BLS/CPR certification preferred; experience with electronic medical records (EMR) a plus

Key Responsibilities

• Provide skilled home health nursing care according to individualized care plans
• Perform comprehensive home health assessments and monitor patient progress
• Administer medications, treatments, and wound care as prescribed
• Educate patients and families on disease management, medication administration, and healthy living
• Coordinate care and communicate changes in condition with the interdisciplinary healthcare team
• Maintain accurate, timely documentation in the EMR and complete required paperwork
