Job Details

• Job Title: Travel Home Health Registered Nurse (RN)
• Setting: Home Health / Home Care Nursing
• Location: Wallingford, CT 06492 and surrounding areas
• Schedule: Monday through Friday, approximately 9:00 AM to 5:00 PM
• Hours: 40 hours per week
• Shift Duration: 8-hour shifts
• Shift Frequency: 5 shifts per week
• Contract Length: 13 weeks
• Weekly Estimated Pay: $2,120 - $2,230
• On Call/Weekend Requirement: None
• Mileage: Reimbursed between visits

Job Requirements

• Associate's degree in nursing required
• Active Registered Nurse license in the state of Connecticut
• Professional certification required

Responsibilities

• Complete skilled nursing visits in the home setting
• Document assessments, interventions, and patient progress in the EMR
• Participate in case conferences and interdisciplinary communication
• Provide safe, compassionate, and patient-centered care
• Support care plans and help promote positive patient outcomes
• Travel within the assigned coverage area to visit patients in their homes
