Job Details

• Weekly Estimated Pay: $2,270-$2,390

• Duration: 13-week contract
• Schedule: 40 hours per week
• Patient Volume: Approximately 25-30 visits per week, including start-of-care visits and revisits
• Work Arrangement: On-site

Job Requirements

• 2+ years of recent and relevant home care nursing or home health nursing experience
• Active Registered Nurse (RN) license in Pennsylvania or eligibility to practice in the state
• Experience with OASIS documentation
• Experience using Epic electronic health record (EHR)
• Strong clinical assessment, care planning, and patient education skills
• Ability to work independently and manage a field-based patient caseload effectively

Responsibilities

• Provide skilled nursing care to patients in the homecare and home health setting
• Complete start-of-care assessments and follow-up revisit appointments
• Perform comprehensive patient evaluations and develop individualized care plans
• Document all patient care accurately using OASIS and Epic
• Monitor patient progress and communicate changes in condition to the care team
• Educate patients and families on treatment plans, medications, and ongoing care needs
• Support high-quality, patient-centered care while maintaining productivity expectations
