• Mileage: Reimbursed between patient visits

Job Requirements

• Active Registered Nurse (RN) license in Pennsylvania
• At least 1 year of nursing experience required
• Home health nursing experience strongly preferred
• Associate degree in Nursing or higher
• Strong communication, time management, and patient education skills
• Ability to work independently and maintain accurate clinical documentation

Responsibilities

• Provide skilled nursing care to patients in a home health setting
• Complete patient assessments, care planning, and progress documentation
• Deliver nursing interventions in accordance with the plan of care
• Coordinate care with the interdisciplinary team to support patient goals
• Educate patients and families on treatment plans, safety, and follow-up care
• Travel between patient homes within the designated service area
• Maintain productivity expectations while delivering high-quality, compassionate care
