• Hours Per Week: 40
• Shift Duration: 8 Hour Days
• Shift Time: 8:00 AM - 4:30 PM
• Weekend Rotation: Approximately once every 6 weeks
• Weekly Estimated Pay: $2,500 - $2,630

Job Requirements

• Active RN license in the appropriate state
• At least 1 year of certified home care or home health nursing experience
• At least 1 year of case management experience
• Ability to travel to assigned patient cases throughout the Newton and North Region service area
• Strong electronic medical documentation and charting skills
• Experience working collaboratively with interdisciplinary care teams

Responsibilities

• Assess patient conditions and identify ongoing care needs in the home setting
• Implement individualized care plans to support safe, effective recovery and patient outcomes
• Provide patient and family education, guidance, and support
• Complete admission processes and case management responsibilities
• Document care thoroughly, accurately, and in a timely manner
• Communicate clearly with the interdisciplinary healthcare team
• Deliver compassionate, patient-centered home health care to patients in their homes
