• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Shift Duration: 8-hour shifts
• Hours per Week: 40
• Weekly Estimated Pay: $2,120 - $2,230
• On-Call Requirement: One weekend on call per month to support start-of-care and on-call visits

Job Requirements

• Active Registered Nurse (RN) license
• Associate degree required
• At least 1 year of nursing experience required
• 1-2 years of home health RN experience preferred
• 1-2 years of HCHB experience required
• 1-2 years of OASIS experience required
• Strong knowledge of home health workflows, patient documentation, and visit coordination
• Excellent communication, organization, and time-management skills

Responsibilities

• Perform OASIS visits for initial assessments, recertifications, resumption of care, and agency discharge
• Provide skilled nursing care to home health patients with a variety of clinical needs
• Complete patient visits across assigned coverage areas in a timely and professional manner
• Support continuity of care through accurate clinical documentation and communication with the care team
• Participate in one weekend of on-call coverage each month for SOCs and on-call visits
• Deliver compassionate, patient-centered care while promoting safety, independence, and positive outcomes
