- Position: Patient Care Coordinator | RN
- Location: Portland, OR 97232
- Setting: On-site travel nursing assignment
- Start Date: 7/5/2026
- End Date: 10/3/2026
- Duration: 13 weeks
- Weekly Hours: 40
- Shift: 8-hour evening/night shift, 4:00 PM to 12:30 AM
- Estimated Weekly Pay: $2,750 - $2,890
- Work Schedule: Weekends and holidays may be required
Registered Nurse Job Requirements
- Active Oregon or Washington RN license, or a compact RN license, on file
- Minimum of 1 year of case management, utilization review, or Patient Care Coordinator experience
- BLS certification required
- Experience with EPIC charting preferred
- Experience working with Medicaid and Medicare populations preferred
- Previous experience in a similar care coordination or clinical support role strongly preferred
Responsibilities
- Coordinate patient care needs and support safe, efficient transitions throughout the care process
- Collaborate with interdisciplinary healthcare team members to help ensure timely patient support
- Manage scheduling, communication, and follow-up related to patient care coordination
- Document accurately in the electronic health record
- Support patients with complex needs in a professional and compassionate manner
- Adapt to floating within a neighboring location as needed
- Maintain a strong focus on quality, organization, and patient satisfaction
