Travel RN Job Details

• Position: Patient Care Coordinator | RN
• Location: Portland, OR 97232
• Setting: On-site travel nursing assignment
• Start Date: 7/5/2026
• End Date: 10/3/2026
• Duration: 13 weeks
• Weekly Hours: 40
• Shift: 8-hour evening/night shift, 4:00 PM to 12:30 AM
• Estimated Weekly Pay: $2,750 - $2,890
• Work Schedule: Weekends and holidays may be required

Registered Nurse Job Requirements

• Active Oregon or Washington RN license, or a compact RN license, on file
• Minimum of 1 year of case management, utilization review, or Patient Care Coordinator experience
• BLS certification required
• Experience with EPIC charting preferred
• Experience working with Medicaid and Medicare populations preferred
• Previous experience in a similar care coordination or clinical support role strongly preferred

Responsibilities

• Coordinate patient care needs and support safe, efficient transitions throughout the care process
• Collaborate with interdisciplinary healthcare team members to help ensure timely patient support
• Manage scheduling, communication, and follow-up related to patient care coordination
• Document accurately in the electronic health record
• Support patients with complex needs in a professional and compassionate manner
• Adapt to floating within a neighboring location as needed
• Maintain a strong focus on quality, organization, and patient satisfaction
