• Shift Duration: 8 hours
• Hours: 40 hours per week guaranteed
• Estimated Weekly Pay: $2,120 - $2,230
• Coverage Areas: Robeson, Hoke, and Cumberland County
• On-Call/Weekend Requirement: Yes
• Productivity Expectation: 30 points

Job Requirements

• Active Registered Nurse (RN) license required
• Associate degree required; BSN preferred if applicable
• At least 1 year of nursing experience required
• Home health nursing experience strongly preferred
• Experience with HCHB documentation preferred
• Strong assessment, communication, and time-management skills
• Ability to work independently while maintaining high-quality patient care
• Comfortable meeting productivity expectations and supporting on-call/weekend coverage as needed

Responsibilities

• Provide skilled nursing care to patients in the home setting
• Complete patient assessments and develop individualized care plans
• Deliver treatments, monitor patient progress, and report changes in condition promptly
• Educate patients and families on care plans, medications, and recovery goals
• Document visits accurately and timely using home health documentation systems
• Coordinate care with the interdisciplinary team to support safe, effective outcomes
• Travel within the assigned coverage area to complete patient visits
• Participate in on-call and weekend coverage as scheduled
