• Weekly Estimated Pay: $2,750-$2,890

• Duration: 13 weeks
• Hours Per Week: 40
• Shift Duration: 8-hour day shifts
• Weekend Rotation: Approximately once every 6 weeks
• Travel Requirement: Ability to travel to patient cases as assigned

Job Requirements

• Active Registered Nurse (RN) licensure
• At least 1 year of certified home care or home health experience required
• At least 1 year of case management experience within a certified home health agency required
• Strong experience with home health documentation and interdisciplinary communication
• Ability to provide care independently while following established policies, procedures, and standards
• Comfortable managing admission processes and ongoing care coordination
• Valid credentials and onboarding requirements must be completed prior to the start date

Responsibilities

• Assess patient conditions and develop appropriate care plans in the home setting
• Provide ongoing education and support to patients and their families
• Perform admission processes and case management responsibilities
• Document care thoroughly and accurately in a timely manner
• Communicate effectively with the interdisciplinary care team
• Deliver care in alignment with organizational philosophy, policy, and standards of community health practice
• Travel to assigned patient cases throughout the service area as needed
