• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• On-Call / Weekend Requirement: Every 3rd weekend and 1 night of on-call per week
• Hours per Week: 40
• Weekly Estimated Pay: $2,120 - $2,230
• Patient Care Setting: Home Health

Job Requirements

• Registered Nurse (RN) license required
• Associate degree in nursing or higher
• Recent home health experience strongly preferred
• Experience with HCHB is a plus
• Strong communication, time management, and documentation skills
• Ability to work independently while covering a defined territory
• Reliable transportation for travel between patient visits; mileage reimbursement is provided between visits

Responsibilities

• Serve as an RN case manager for a designated territory
• Complete skilled routine visits, recertifications, discharges, and start-of-care/resumption-of-care visits
• Deliver compassionate, 100% patient-focused care in the home environment
• Coordinate care and collaborate with interdisciplinary team members to support positive patient outcomes
• Manage a patient caseload with productivity expectations while maintaining quality care
• Cover assigned patients and assist teammates as needed on days off
• Follow clinical standards, documentation requirements, and care plans accurately and in a timely manner
• Participate in rotating on-call and weekend coverage as scheduled
