Job Details

• Start Date: 10/18/2026
• Duration: 13 weeks
• Schedule: 8-hour day shifts, 40 hours per week
• Weekend Rotation: Approximately once every 6 weeks
• Weekly Estimated Pay: $2,500 - $2,630

Registered Nurse Job Requirements

• Active Registered Nurse (RN) license
• At least 1 year of certified home care experience required
• At least 1 year of case management experience required
• Ability to travel to assigned patient cases as needed
• Strong electronic documentation skills and comfort with interdisciplinary communication
• Experience working within community health or home-based care settings preferred

Home Health Nursing Responsibilities

• Assess patient conditions and provide skilled nursing care in the home setting
• Develop and implement individualized care plans based on patient needs
• Perform admission assessments and case management responsibilities
• Provide ongoing education and support to patients and their families
• Document care thoroughly and in a timely manner using electronic systems
• Communicate effectively with the interdisciplinary care team to support coordinated care
• Follow organizational policies, standards of practice, and community health guidelines
