- Travel Territory: Up to 75 miles from the assigned care center
- EMR: HomeCare HomeBase (HCHB) / Pointcare
- Documentation: Completed within 24 hours of each visit
- On-Call: Potential rotating on-call coverage
- Dress Code: Scrubs
- Productivity Expectation: 30 points per week

Job Requirements
- Active Registered Nurse (RN) license required; license must be in hand
- Minimum of 1 year of Adult Home Health or Hospice experience required
- Recent home health specialty experience required
- Current BLS/CPR certification required
- Proficiency with HomeCare HomeBase within the last 6 months required
- OASIS proficiency required for RNs, or willingness to complete retraining during compliance if needed
- Reliable transportation, valid driver's license, and auto insurance required
- Comfortable traveling daily to patient homes and attending weekly clinic meetings
- Willing to travel up to 75 miles from the assigned care center
- Open to rotating on-call coverage as needed
Responsibilities
- Provide skilled nursing care in patients' homes according to physician orders, agency policies, and individualized care plans
- Coordinate and direct home health nursing services to support quality patient outcomes
- Perform start-of-care, wound care, follow-up visits, and other home health visits
- Complete patient assessments, care planning, and timely clinical documentation
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as assigned
- Manage a daily caseload while maintaining productivity expectations
- Collaborate with the interdisciplinary healthcare team to promote continuity of care
- Travel between patient homes while maintaining professionalism, efficiency, and excellent communication
