Job Requirements

• Active Registered Nurse (RN) license in Pennsylvania
• Associate’s degree required; additional nursing education welcomed
• Home health or home-based care experience preferred
• Experience with OASIS assessments strongly preferred
• Skilled in care plan development and return patient visits
• Experience with IV therapy, wound vacs, wound care, disease education, and venipuncture
• Strong communication, assessment, and documentation skills
• Professional certification required
• Scrub color: Royal blue or black

Responsibilities

• Complete home health nursing visits with a focus on quality, compassionate patient care
• Perform OASIS assessments to support accurate patient evaluation and care planning
• Develop and update individualized care plans based on patient needs and clinical findings
• Provide return visits and ongoing follow-up care in the home setting
• Deliver patient and family education related to disease management and treatment plans
• Support wound care needs, including wound vac management when appropriate
• Administer IV-related care and perform venipuncture as needed
• Work collaboratively to coordinate safe, effective care across the service area
• Maintain accurate clinical documentation and timely communication
• Represent a high standard of professionalism while caring for patients in the community
